I am new to working with DFFS. I installed the DFFS July 2013 version to a test site. I followed the setup instructions in your video.
I added the DFFS Loader by SPJSBlog.com column to my list. Then I created 3 columns in the list and opened the new item form and in turn clicked the Enhanced with DFFS link. I then added three tabs. Seems straightforward enough. Saved filling in the fields and closed. Everything saved as normal.
The next time I opened the new item form and clicked on the Enhanced with DFFS I receive the HierarchyRequestError.
Attached is a screen shot of the message and the F12 Developer Tools window. Any guidance in resolving this would be appreciated.

Which version of SharePoint and DFFS are you using?
I believe I have seen this issue reported earlier, but cannot recall what fixed it. If you could send me some more information I’ll see if I can find the solution.
PS: If you are using SP 2007 or 2010 – try disabling “Compatibility mode” in the browser if this is turned on.
