Emails missing To and CC line

I was working with MDFFS version 1.1.13.0, and recently upgraded to 1.1.14.0, then 1.1.15.0, then 1.1.16.0, in a matter of a couple of days. Emails quit working in version 1.1.15.0 and 1.1.16.0. They rendered without any To line or CC line.

I’m not able to recreate this either – can you export the config for your form and send it to me by email? – I need to see the config to try to understand what is going on.
