Cascading dropdowns

I am trying to set up multi-select cascading dropdowns, and hoping you can help.

In the setup, I have the GUID in the first field and then {currentSite} in the second field. Then, I have the list fields separated by commas. Everything looks exactly how it should be.

However, when I save the form, I get a message that says:
[Cascading dropdowns]: An error occurred.
Please enter the setup and verify that the list GUID or display and name the BaseURL is correct.

Could you post a screenshot of the cascading dropdown setup so I can take a look?
