An error occurred, please review setup

Hi Alex,
I’m getting this error with my Cascading Dropdown setup:
“An error occurred, please review setup”.
I have a source list located in the same site named “CascadingDropdownSource” that has 3 fields Level1 (internal name: Title), Level2 and Level3.
I created 3 “Single line Text fields” named cddLevel1, cddLevel2 and cddLevel3 to match up the 3 fields in the source list.
I have attached all the screenshots with the error, Cascading Dropdown configuration, source list and current list field settings.
Appreciate any help you can provide with this issue.

Hi,
You have the wrong BaseUrl – use this:

/sandbox/artestsandbox

or just

{currentsite}
