Worklog Entries Duplicating

Hi All
We have a list built as Service Requests that has a Worklog field for staff to update. Our problem is that some worklogs are filling up with multiple duplications of entries. It can be from the assigned staff updating or the requester updating. Sometims it happens when an attached document is opened and no worklog entry is entered.
I can delete the entries from the version history but obviously not a lomg term solution.

What kind of field is this? – a normal multiline field with append, or is it made using Custom JS / DFFS rules?
